NTA NEET UG 2026 Admit Card

National Testing Agency (NTA)

Must include all pages with passport photo pasted and left-hand thumb impression before reaching the centre.

Must be signed by candidate & parent; invigilator signature during exam is verified.

Admit Card Proforma with Postcard Photograph

Proforma sheet bearing 4x6 inch postcard photograph pasted and cross-signed by invigilator.

Class 10 (Matriculation) Certificate & Marksheet

School Examination Board (CBSE/ICSE/State)

Serves as statutory proof of Date of Birth (DoB) and candidate's mother & father name spelling.

Name spelling must match exactly with Aadhaar and NEET Admit Card.

Class 12 (Senior Secondary) Marksheet & Certificate

School Examination Board

Must prove minimum qualifying aggregate in Physics, Chemistry, Biology (50% UR, 40% OBC/SC/ST, 45% PwBD).

Candidates who took Biology as an additional subject must produce both main and additional marksheets.

Provisional Seat Allotment Letter

MCC (DGHS) or State Counselling Portal

Generated from official portal after round allotment showing allotted college, quota, and category.

Original Government Photo Identity Proof

Government of India

Aadhaar Card, Passport, PAN Card, Driving License, or Voter ID. Digital DigiLocker is secondary.

Passport Size Photographs (8 to 12 copies)

Self

Must be identical to the photograph uploaded during NEET UG online application with name & date.

Gap Year Affidavit (Stamp Paper Notarized)

Notary Public / Sub-Registrar

Sworn affidavit on ₹50 or ₹100 non-judicial stamp paper declaring exam prep and no criminal/unlawful record.

Mandatory in all State GMCs and private colleges for candidates with 1+ year gap after 12th.

Original Migration Certificate

School Board or Previous University

Issued by CBSE, ICSE, or State Board. If enrolled in B.Sc, university migration is required.

Transfer Certificate (TC) / School Leaving Certificate

Last Attended School or College

Original copy retained by the admitting medical college until completion of MBBS.
